Work Permits

All children ages 14 through 18 are required to obtain work permits when employed.
The Randolph Public Schools, Family Resource Center is an issuing authority.
Please complete the work permit application below & Please follow these guidelines when completing the application process.
Section 1 gets filled out by the employer ONLY and they must sign and date it. Do NOT fill this out yourself.
Section 2 is for a doctor/physician signature; students who are 14 and 15 MUST get a Physician’s signature on the work permit application. This section is not required for students age 16-17.
Section 3 is for a parent/guardian signature; ALL students need to have this to complete the application.
**A form of ID is required. The student applying for the work permit needs to sign it in person at the FRC office.
You will NOT be issued a permit without one of the following identification documents:- Birth Certificate
- Social Security Card
- Passport
- Permanent Resident Card
- Learner’s Permit
- Driver’s License
- Mass ID
- Mass Health Card
- School ID
When the application is complete, please email the Family Resource Center: Registrationoffice@randolph.k12.ma.us, with the completed Permit form and a request for an appointment for an online ZOOM meeting. You will be required to show your ID during the ZOOM video call.

The work permit form is fillable online, following the instructions to fill, save, and submit the form:
~Open form in Google Chrome Browser
~Fill in the required information
~Click the print icon in the upper right corner, and choose "Save as PDF"
~Attach the form in an email to Registrationoffice@randolph.k12.ma.us
